SC Certificates of Immunizations update

Effective May 1, 2023, SC Health Departments have a new statewide policy for issuing SC Certificates of Immunizations and releasing personal shot records. This new procedure pertains to any current or incoming LCSD student who requests an immunization record from any SC Health Department.
A SC Certificate of Immunization or personal shot record can only be released to a person 16 years of age or older or to their parent/guardian (if less than 18 years of age) with written consent.

The following documentation is required when requesting a SC Certificate of Immunization or personal shot record:
a. The Authorization to Release Immunization Registry Information (This form may only be obtained at a Health Department)
b. A government-issued photo ID

Once the required documentation is received, there will be a 5-7 business day waiting period for the requested document to be issued.

DHEC staff will not release immunization information from the personal shot record or a SC Certificate of Immunization to schools or childcare facilities.
School nurses may not request immunization records for a student.
